DISEASE PROGRESSION:
The infection begins on a specific finger or toe and spreads inward along an anatomically
logical path — same-side digits first, then the hand or foot, then the arm or leg, before
crossing to the opposite side. The full course takes approximately 14 in-game days untreated.
Stage 1 — Fever and chills: fatigue and pain, no visible tissue loss yet
Stage 2 — Digits blackening: fingers and toes begin to die and fall away
Stage 3 — Extremities consumed: hands and feet are lost
Stage 4 — Limbs consumed: arms and legs consumed
Stage 5 — Consuming the core: organ failure and brain death
A red threat letter fires when a pawn enters the terminal stage — death is imminent
and the player will not miss it.
TREATMENT
High-quality medicine can reverse the disease if tending begins early.
The pawn's medicine policy must allow medicine — set to None, the disease will progress unchecked.
Glitterworld medicine — cures in ~6 days
Industrial medicine — cures in ~9 days
Regular medicine — cures in ~18 days
Herbal medicine — very slow recovery (~45 days)
Penoxycyline prevents infection entirely if taken before exposure.
SURGERY
Emergency debridement can halt the disease before it reaches the torso:
Early stage (digits affected) — the hand or foot is amputated to stop the spread
Mid stage (hands/feet affected) — the arm or leg is amputated
Success chance scales with medicine quality (30% herbal to 95% glitterworld).
The limb is lost regardless of outcome. On failure the disease re-anchors to the
next part inward and accelerates — a failed surgery is not a safe option.
DOCTOR TRANSMISSION
Doctors who tend infected pawns risk contracting the disease themselves.
Infection chance per tending operation scales with room cleanliness:
Filthy room — 20% chance Clean room — 3% chance
Dirty room — 12% chance Sterile room — 1% chance
Normal room — 6% chance
Pawns resting in medical beds within 4 tiles also risk infection at 40% of the
doctor rate. Walls block transmission — line of sight is required.
Build a proper isolated hospital ward to protect your medical staff.
ENVIRONMENTAL EFFECTS
Infected pawns generate blood and wound discharge (filth) at an escalating rate
as the disease progresses, making rooms dirtier and raising transmission risk.
Healthy pawns sharing a room with an infected colonist suffer mood penalties
that scale with the disease stage — from mild unease to profound distress.
The infected pawn radiates a beauty penalty that compounds room mood effects.
BIOME DISTRIBUTION
Most common in tropical swamps and rainforests. Rare in temperate swamps
and arid shrublands. Absent from cold and dry biomes.
Compatible with RimWorld 1.6 (Odyssey DLC supported).
Safe to add to existing saves.

- Added Mod Settings entry (Options → Mod Settings → Necrotic Fever)
with a toggle to disable the tropical/swamp biome restriction.
When disabled, Necrotic Fever outbreaks can occur on any map.
- Setting takes effect immediately — no restart required.
- Default behaviour is unchanged: biome restriction is enabled by default,
matching all previous versions of the mod.
- Biome registration logic moved from XML patch to C# runtime management.
BiomeRegistration.xml has been removed.
